The Internal Affairs Supervisor falsely banned someone, after they got terminated in a whole different server, the banned person has been unbanned, but the staff member hasn't been punished.

We sincerely apologize for this. The staff member in question has been relieved of their duties. This process took longer than usual, as we needed to conduct a thorough investigation before taking action.

A high ranking staff member false banned someone after they got terminated from a department in another server. When the person was found to have false banned the ban was revoked but the staff wasn't punished.
